29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: On or about April 20, 2022, and at times prior thereto, at the jobsite located at 1501 Scripture Street, Denton, TX 76201, the nine (9) tiered scaffold system did not have guard rails installed along all open sides and ends of platforms during the following instances: a) The employee(s) occupied seventh tier of the scaffold system was missing end rails; b) The employee(s) occupied eighth tier of the scaffold system was missing end rails.
