29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: a) At the Zerand Cutting and Creasing line, on or about 11/18/2021: A junction box did not have a cover to enclose live parts. b) At the Zerand Cutting and Creasing line, on or about 11/18/2021: Control boxes were not used per listing and label as the knockouts were removed and not sealed. c) At the Zerand Cutting and Creasing line, on or about 11/18/2021: Spliced wires were not protected by a junction box. d) At the Zerand Cutting and Creasing line, on or about 11/18/2021: Wires from the control boxes were not installed with strain relief, nor enclosed in conduit. e) At the Zerand Cutting and Creasing line, on or about 11/18/2021: Wires were used without secondary insulation.
