29 CFR 1910.106(c)(3): Above ground piping with threaded joints used for transfer of flammable liquid was not secured to prevent disengagement at the fitting and the piping system was not so designed that any spill resulting from such disengagement could not unduly expose persons, important buildings or structures, and could be readily controlled by remote valves: (a) On or about February 27, 2021 in the process area near the Stripper Column in Unit #6 at the Lion Oil Refinery in El Dorado, AR, workers were exposed to fire and explosion hazard when above ground threaded piping used for transferring Naphtha between Units #6 and #9 was not secured to prevent disengagement of threaded joints at the fittings and/or so designed that any spill resulting from such disengagement could not unduly expose persons, important buildings or structures. Additionally, the piping was not readily controlled by remote valves.
