29 CFR 1910.137(c)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests. Test voltages and/or the maximum intervals between tests were not conducted in accordance with Table I-4 and Table I-5: a. PM #9, Drive Room 2- On or about November 27, 2019, an employee deenergizing the 2400 volt fused disconnect for the #2 Jones Refiner was not protected from electric shock where W.H. Salisbury & Co. class 2 17000 volt insulated rubber gloves used to deenergize the equipment were not tested within 6 months of being placed into service. b. PM #9, Drive Room 1- An employee deenergizing the 2400 volt fused disconnect for the Spare Press Vacuum Pump was not protected from electric shock where W.H. Salisbury & Co. class 2 17000 volt insulated rubber gloves used to deenergize the equipment were not tested within 6 months of being placed into service.
