29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations: 29 CFR 1910.303(b)(1)(iv): Electrical insulation. a. On or about April 17, 2018, in the welding area, the outer protective insulation, which was covered with electrical tape, on the cord for the DeWalt Angle Grinder (model: DWE402N) was damaged. b. On or about April 17, 2018, in the welding area, the outer protective insulation, which was covered with electrical tape in one location, on the extension cord used to power the DeWalt Angle Grinder (model: DWE402N) was damaged in a couple of locations and had an exposed bare conductor. c. On or about April 17, 2018, in the office, the outer protective insulation on the cord for the Bissell Vacuum Cleaner (model: 92L3-W) was damaged in a couple of locations.
