29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee: Roof: Employees were exposed to falls of greater than forty (40) feet to the concrete floor below while using single leg anchor points that were not installed in accordance with the manufacturer's instructions, in that they were installed on a ribbed surface rather than a flat or pitched surface, the fasteners used to install them were not those specified in the instructions, and the quantity of fasteners used was less than the quantity required in the instructions.
