29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. Warehouse: One orange temporary flexible cord, approximately 50 foot, was not provided with a grounding pin. The pin was broken off from male plug. The plug was plugged into a wall mounted receptacle to supply 120 volts to Junghieinrich electric hand pallet truck. Employees were exposed to electrical shock hazard.
