29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment: On, or about, May 25, 2016, in the following locations, openings were left in electrical panels that exposed live parts: A. Mold Room: An opening was left in the Sylvania panelboard above blanks located on the left side of the box. B. Power House: An opening was left uncovered between overcurrent devices at positions 3 and 4 on Switchboard LC2.
