29 CFR 1917.125: Ditches, pits, excavations and surfaces in poor repair were not guarded by readily visible barricades, rails or other equally effective means: a) On or about August 28, 2014, in the rail shed, a grate covering an in-floor drainage channel was missing. The missing grate resulted in an area measuring 37 inches by nine inches being open. Employees were exposed to a slip/trip/fall hazard.
