29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a. On or about July 31, 2014, at the foam department of the above addressed worksite, a fan (i.e. Chicago, 24-inches) lacked a ground prong on its attachment plug, exposing employees to an electrical hazard. b. On or about July 31, 2014, at the foam-in-place department of the above addressed worksite, three fans lacked a ground prong on its attachment plug, exposing employees to an electrical hazard. c. On or about July 31, 2014, at the glue department of the above addressed worksite, an electrical extension cord lacked a ground prong on its attachment plug, exposing employees to an electrical hazard. d. On or about August 21, 2014, at the foam-in-place department of the above addressed worksite, a digital scale (i.e. EAS Counting Scale, 36X-005KG-50X.01LB) lacked a ground prong on its attachment plug, exposing employees to an electrical hazard. e. On or about August 21, 2014, at the door department of the above addressed worksite, a fan (i.e. TPH Industrial) lacked a continuous path to ground, exposing employees to an electrical hazard. f. On or about August 21, 2014, at the door department of the above addressed worksite, a fan (i.e. Dayton) lacked a continuous path to ground, exposing employees to an electrical hazard.
