29 CFR 1910.1030(h)(5)(i)(A): The employer did not maintain a sharps injury log that contained at a minimum the type and brand of device involved in the incident: (a) On or about 7/15/2014 at The New York and Presbyterian Hospital dba NewYork-Presbyterian Hospital/ Columbia University Medical Center. the employer failed to ensure that the type and brand of device involved in needlestick injuries was documented in the sharps injury log. This included but was not limited to entry 2014-05218 which occurred on 10/3/2014.
