29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets of other forms of approved enclosures, or other means listed under this provision: a) Next to the soap storage bin - On or about July 25, 2012, an electrical control box was left opened and energized. b) On line 3 - On or about July 25, 2012, a loose cover plate exposed electrical contact in the gate heater.
