29 CFR 1910.119(e)(3)(ii): The process hazard analysis did not identify any previous incident which had a likely potential for catastrophic consequences in the workplace: a- Hess Port Reading Refinery - The 2000 PHA for the FCCU did not consider the P- 501A Main Feed Pump fire, incident no. 97-085 which occurred on 10/10/97. Condition noted on or about 02/20/09. b- Hess Port Reading Refinery - The 2005 FCCU PHA did not consider an incident (no. 04-66 dated 12/19/04) involving a hydrocarbon release from the top of D-909 which impacted the unit. Condition noted on or about 03/13/09.
