29 CFR 1910.146(k)(3)(i): Each authorized entrant did not use a chest or full body harness, with a retrieval line attached at the center of the entrant's back near shoulder level, or above the entrant's head: On or about August 1, 2025, and at times prior, on a site located at 4529 Hypoluxo Road, Lake Worth, Florida, the employer did not ensure employees entering an underground fuel tank containing hazardous chemical vapors such as, but not limited to Benzene, Toluene and Merican 9505 Unsaturated Isophthalic Resin, used a chest or full body harness attached to a retrieval line.
