29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: b) On or about April 28, 2025, and at times prior, the employer, Powdr - Copper Mountain LLC DBA Copper Mountain Resort, did not ensure that one or more methods of machine guarding was provided to protect employees from hazards such as points of operation, ingoing nip points and rotating parts. Employees were required to perform maintenance work on lift support and drive system components that included accessing catwalk platforms on chair lifts including, but not limited to, the American Flyer and the Kokomo Express. While positioned on the catwalks, employees were in close proximity to or performing work on unguarded drive system sheaves. These conditions exposed employees to amputation and caught-in hazards. c) On or about April 28, 2025, and at times prior, the employer, Powdr - Copper Mountain LLC DBA Copper Mountain Resort, did not ensure that one or more methods of machine guarding was provided to protect employees from hazards such as points of operation, ingoing nip points and rotating parts. Employees were required to perform maintenance work on lift drive system components that included accessing platforms attached to lift housings on chair lifts including, but not limited to, the American Flyer, the Super Bee, the American Eagle, and the Excelerator. While positioned on these platforms, employees were in close proximity to or performing work on unguarded drive system sheaves. These conditions exposed employees to amputation and caught-in hazards. d) On or about April 28, 2025, and at times prior, the employer, Powdr - Copper Mountain LLC DBA Copper Mountain Resort, did not ensure that one or more methods of machine guarding was provided to protect employees from hazards such as points of operation, ingoing nip points and rotating parts. Employees were required to perform maintenance operations on carriages that were part of the tensioning system in lift housings on chair lifts including, but not limited to, the American Flyer and the American Eagle. During these work operations, employees were in close proximity to vertical guard rails directly adjacent to carriages that were moving horizontally. These conditions exposed employees to caught-in hazards.
