29 CFR 1926.1053(b)(15): Ladder(s) were not inspected by a competent person for visible defects on a periodic basis and/or after an occurrence that could have affected their safe use: a) Front Roof Access Ladder- On or about September 21, 2024, employees working on a roof were exposed to a fall hazard of approximately eighteen (18) feet, four (4) inches from the use of a portable ladder that had not been inspected and had deficiencies such as but not limited to bent rungs and side rails. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
