29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder: Jobsite: On or about May 27, 2021, the extension ladder being used for access to the rear roof of the building was not set at an angle approximately one-quarter of the working length of the ladder where the ladder was approximately 11 feet high and the base was out over 10 feet from the building.
