29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Warehouse Area - On or about 03/06/2020, the employer exposed employees throughout the production and/or warehouse areas to struck-by hazards in that untrained employees were authorized to operate the company's Toyota Electric Forklift Truck (model# 9BRU18 s/n 9BRU18-396240), Toyota Order Picker (model# 8BPU15), Skyjack Scissor Lift (model# SJ3226), and Toyota Forklift Truck (model# 8FGCU25 s/n 96871).
