29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: The employer is failing to protect employees from electrical shock hazards associated with warehouse operations. This was most recently documented on or about 05/31/2019 at the Shamrock Tire & Auto Service, Inc. dba Tire Dealers Wholesale facility located at 3003 S. Broadway St. Wichita, Kansas 67216. A splice in the extension cord that powered the light for the auger equipment did not return the cord to the state in which it was approved.
