29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about 2/14/2019, employees throughout the facility were exposed to electric shock hazards at locations including but not limited to: a) the weld shop, equipment such as but not limited to the lift tables and hand tools were receiving power from flexible cords plugged into metal electrical junction boxes with prepunched knockouts used as extension cords and not mounted to a rigid surface. b) hydraulic press areas, equipment such as but not limited to the radio and hand tools was receiving power from flexible cords plugged into metal electrical junction boxes with prepunched knockouts used as extension cords and not mounted to a rigid surface. c) Buffalo pedestal drill press, was receiving power from flexible cords plugged into metal electrical junction boxes with prepunched knockouts used as extension cords and not mounted to a rigid surface.
