On or about 10/29/2018, in the site work area of the Traffic Barricade Type H (TBH) form equipment located outside of the facility at 220 Cargill Road Connector, Macon, GA: Employee(s) used four (4) hook wire rope sling(s) without a permanently affixed identification marking as prescribe by the manufacturer, to lift the CAN cover on and off the Traffic Barricade Type-H (TBH) form equipment.
