29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about June 13, 2017, at the plant at 617 Art Street, in the detail aisle, the green hydraulic punch identified by Hirschfeld equipment number 86024, was not guarded. Employees were using it to punch holes. Employees exposed to the hazard of being caught-in the punch. b) On or about June 13, 2017, at the plant at 617 Art Street, in the detail aisle, the Kingsland hydraulic punch was not guarded. Employees were using it to punch holes. Employees exposed to the hazard of being caught-in the punch. c) On or about June 14, 2017, at the plant at 617 Art Street , in the heavy fab aisle, the MAS horizontal drill was not guarded. Employees were using it to drill holes. Employees exposed to the hazard of being caught-in the rotating and protruding drill bit. d) On or about June 14, 2017, at the plant at 617 Art Street, in the heavy fab aisle, the white Weiler Holoubkov horizontal drill was not guarded. Employees were using it to drill holes. Employees exposed to the hazard of being caught-in the rotating and protruding drill bit. e) On or about June 14, 2017, at the plant at 617 Art Street, in the detail aisle the Accurpress brake was not guarded when being used. Employees were using it to bend steel. Employees were using the foot pedal and not the two hand safety device installed on the machine. Employees exposed to the hazard of being caught-in the press brake. f) On or about June 14, 2017, at the plant at 617 Art Street, in the girder laydown aisle, the Rojar milling machine was not guarded. Employees were using it to shave metal off items to make them smaller. Employees exposed to the hazard of being caught-in the rotating milling head and struck-by flying metal chips. g) On or about June 14, 2017, at the plant at 617 Art Street, girder laydown aisle, the Wynson press brake was not guarded when being used. Employees were using it to bend steel. Employees were using the foot pedal and not the two hand safety device installed on the machine. Employees exposed to the hazard of being caught-in the press brake. h) On or about June 14, 2017, at the plant at 2901 Old Ballinger Highway, in girder plant, the Kaukauna horizontal drill was not guarded. Employees were using it to drill holes. Employees exposed to the hazard of being caught-in the rotating and protruding drill bit. i) On or about June 14, 2017, at the plant at 2901 Old Ballinger Highway, in girder plant, unnamed milling machine was not guarded. Employees were using it to shave metal off items to make them smaller. Employees exposed to the hazard of being caught-in the rotating milling head and struck-by flying metal chips.
