29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven feet or less above floors or platforms were not enclosed. On or about 1 May 2017, and at times prior thereto, employees performing box making activities on the large gluer were working in close proximity to exposed sprocket wheels and chains less than seven feet from the floor, exposing the employees to hazard of being struck by or caught in the moving chains and sprocket wheels. A) Employees feed boxes into the gluer while standing and kneeling directly next to an unguarded vertical chain which raises and lowers the feed table. B) Employees adjust the glue head on the machine in the vicinity of an unguarded diagonally running chain which powers the rollers.
