29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) Throughout facility: The employer did not conduct evaluations every three years to determine employees competency to operate the powered industrial vehicles in the facility, such as, but not limited to, a Crown electric pallet jack model WP3035-45 serial 7A296639, a Yale stand up forklift model 5021352-01 and a Nissan sit down forklift, on or about 8/9/16. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
