29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: a) Rail mill stacking bed: On or about March 4, 2016, and at times prior, workers were exposed to caught-in hazards where the controlled entry barrier chain was not attached and not capable of being attached to prevent access to stacking bed end sprocket in-going nip points. b) Rail mill stacking bed: On or about March 4, 2016, and at times prior, workers were exposed to machinery hazards where chain and sprocket guards were not installed along the walkway.
