29 CFR 1910.272(k): Receiving pit feed openings in the grain handling facility, such as truck or railcar receiving pits, were not covered by grates which had a maximum opening width of 2 1/2 inches: Plant, Receiving Area: On or about February 8, 2016 and times prior, employees were exposed to tripping and fall hazards caused by grates in the receiving area being 4 inches wide by up to 8 inches long.
