29 CFR 1910.119(m)(1): The employer did not investigate each incident which resulted in, or could reasonably have resulted in a catastrophic release of highly hazardous chemical in the workplace. a) On or about 05/16/16, in the cylinder department; employer did not conduct an incident investigation when pressure was detected in SO2 Expansion Chamber #1, indicating the rupture disc had failed exposing employees to SO2 vapors and or liquid.
