29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: A.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA 15824, on or about November 23, 2015 and times prior thereto: Employees perform service and maintenance on equipment including, but not limited to, powdered metal presses. Service and maintenance included, but was not limited to, cleaning and setup. The company does not have a lockout tagout program consisting of elements including, but not limited to, procedures, periodic inspections, authorized employee training, and affected employee training. In addition, the company did not have a lockout tagout program consisting of lockout devices including, but not limited to, locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, ram blocks or other hardware for isolating, securing or blocking of machines or equipment from energy sources.
