29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: a) Perma-Cast; On or about March 5, 2015 and at times prior thereto the employer exposed employees to caught-by / amputation hazards in that employees were allowed to walk and work in close proximity of unguarded projecting shaft ends that were not smooth and projected more than one and half times the diameter of the shafts on the aluminum round no taper machines 330 and 331. b) Perma-Cast; On or about March 5, 2015 and at times prior thereto the employer exposed employees to caught-by / amputation hazards in that employees were allowed to walk and work in close proximity of unguarded projecting shaft ends that projected more than one and half times the diameter of the shafts on the mixing tanks in the bulk mix room.
