29 CFR 1910.261(k)(2)(iv): All exposed shafts were not guarded as required: a) On or about 12/10/2014 #2 Tissue machine supply fan on mezzanine had an exposed shaft with a disc attached to it approximately 18" from the frame of the machine. b) On or about 12/10/2014 #2 slusher tank had approximately 6" of the broke chest agitator drive shaft exposed. c) On or about 12/10/2014 #5 White water chest in #6 building basement had approximately 6" of drive shaft exposed for the agitator.
