29 CFR 1926.652(d)(2): Manufactured materials and equipment used for protective systems shall be used and maintained in a manner that is consistent with the recommendations of the manufacturer, and in a manner that will prevent employee exposure to hazards. a) On or about April 25, 2024, the employer failed to ensure that the trench boxes in place were rated for the soil type and depth of the excavation per manufacturer's specifications. As a result, employees were exposed to cave-in hazards while working in a 16 foot excavation.
