29 CFR 1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): (a) Fermentationexperts, Inc. at 1201 North Ellis Road, Sioux Falls, SD 57107: On or about and at times prior to March 24, 2022, the employer did not ensure that electrical cords were removed from service that were damaged in that a plug in light in the basement area was damaged and spliced and not removed from operation. This condition exposed employees to shock and electrocution hazards associated with the damage electrical cord.
