29 CFR 1910.179(j)(2)(iii): Hooks with deformation or cracks. Visual inspection daily; monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected. For hooks with cracks or having more than 15 percent in excess of normal throat opening or more than 10�° twist from the plane of the unbent hook refer to paragraph (l)(3)(iii)(a) of this section. a. Throughout All Facilities - On or about September 23, 2014, the employer failed to perform a monthly inspection of hooks, with signed reports, which were not completed, and not limited to the following equipment: 4400 S. Packers - North Receiving Bay - Abell-Howe, Model TR, SG, DM, Serial # 83-17963-1, 10 Ton Crane 4400 S. Packers - Contryx, Model GC2-16, Serial # 38929, 2 Ton -C Gantry Crane 4400 S. Packers - Contryx, Model GC2-16, Serial # 31444, 2 Ton - D Gantry Crane 4400 S. Packers - Unknown, E Gantry Crane 4300 S. Packers - Contryx, Model GC2-16, Serial # 32615, 2 Ton - X Gantry Crane Employees were thereby exposed to fractured bones, and crushing injuries due to damaged hoisting hooks.
