29 CFR 1910.134(l)(1): The employer did not conduct evaluations of the workplace as necessary to ensure that the provisions of the current written program were being effectively implemented and that it continued to be effective. a. MCM Precision Castings, Inc. located in Weston, Ohio: On or about July 17, 2014, the employer did not ensure the respirator program was regularly evaluated to determine if it was being effectively implemented. Medical evaluations were not being conducted when employees were required to wear dust masks and then later powered air purifying respirators to protect themselves from substances such as metal fumes and silica dust.
