29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) Production Area - Approximately thirteen employees who operated the facilitys powered industrial trucks including, but not limited to two Caterpillar propane sit-down forklifts (model T120C, serial number 55L665 and model T60B), and two electric stand-up forklifts (one Raymond model 21R30TT, serial number 021.85.01050 and one Deka model LT 150-20, serial number 38106), and who were trained in 2007, were not re-evaluated at least once every three years since that time. On or about 6/27/13. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
