29 CFR 1910.119(h)(2)(iv): The employer did not develop and implement safe work practices consistent with paragraph (f)(4) of this section, to control the entrance, presence and exit of contract employers and contract employees in covered process areas: a) Sunoco Marcus Hook Refinery - The employer did not ensure that contract employees followed established procedures for entering and exiting the 10 Plant, as evident by the fact that the Visitor's Logs for the 10 Plant, including those from approximately 12/04/2008 to approximately 2/20/2009, contained numerous deficiencies, including the failure to log the time in, time out, specific location in the unit, and date, exposing employees to the potential of not being located in the event of an emergency, on or about 2/20/2009.
