29 CFR 1910.178(l)(3)(i)(G): Fork and attachment adaptation, operation, and use limitations: On or about March 18, 2025, the employer did not ensure employees utilizing a Clark Equipment forklift, Model No. GCX25, Serial No. GX230-0173-8805K0F, equipped with a front end telescoping boom lifted, used in slab handling operations where trained on the attachment's adaptation, operation, and use limitations, thus exposing employees to crushing and struck-by hazards.
