29 CFR 1926.451(e)(6)(vi): Integral prefabricated scaffold access frames being used for access had a spacing greater than 16 3/4 inches: a) Jobsite: 2250 E Cambria St, Philadelphia, PA 19134 - On or about August 17, 2024, the employer did not ensure the maximum spacing between the rungs on the integral prefabricated scaffold access frames of the supported tubular welded frame scaffold did not exceed 16 3/4 inches. The spacing between the rungs on the integral prefabricated scaffold access frames used by employees to access the scaffold was approximately 20 inches, thereby exposing employees to fall hazards.
