29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy. a.) On or about January 10, 2024, in the saw area, an electric wall box was not equipped with a cover. b.) On or about January 10, 2024, in an aisle leading to the main east exit, the cover to an electric wall box was missing a screw and hanging from the bottom of the junction box. c.) On or about January 10, 2024, in welding booth #5, the cover to an electric wall box was not effectively closed and the cover was missing a screw and pushed to the side with wires outside of the box.
