29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: Central Paper Stock, Inc. located at 6665 Jonas Place in Berkeley, MO 63134: The employer is failing to inform employees of the presence of permit-required confined spaces. On or about 10/26/2021, the employer failed to inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces, who were entering permit-required spaces on machines such as, but not limited to, the following: a) The large "Harris Selco" baler (Model: HLO-8221AR-150; Serial No: 39770079) on the paper side of the plant; b) The small "Harris Selco" baler (Model:L125-2-10/8 ; Serial No:08138825 ) on the paper side of the plant; c) The "Retech" book grinder (Model: RG 52; Serial No: 7739101) on the paper side of the plant; and, d) The plastic "Badger" baler (Model: BADGER L-100S-2-10/7; Serial No: 8658) on the plastic side of the plant. NOTE: A sign reading DANGER -- PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign.
