29 CFR 1926.1204(e)(2): Atmospheric hazards were not continuously monitored. The employer did not demonstrate that the equipment for continuously monitoring a hazard is not commercially available or that periodic monitoring is of sufficient frequency to ensure that the atmospheric hazard is being controlled at safe levels: a) Standard Pipe Services, LLC temporary site located at 111 Forge Ct, Malvern, PA 19355: The employer did not ensure that storm sewer pipes were continuously monitored for atmospheric hazards while employees worked in the permit-required confined space. Employees entered and stayed in the permit-required confined space for approximately 8 hours per day, exposing them to atmospheric hazards. The condition was noted on or about September 17, 2020.
