29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs seven feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt: The employer is failing to protect employees from caught-in and amputation hazards during foam filling operations. This was most recently documented on or about 05/31/2019 at the Shamrock Tire & Auto Service, Inc. dba Tire Dealers Wholesale facility located at 3003 S. Broadway St. Wichita, Kansas 67216. The employer did not provide guards around the belt for the auger which were located approx. 72 inches off the floor.
