29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy. On or about October 22, 2018, and at times prior there to, in the old break room building, employees used electrical outlets without cover plates exposing them to electrical shock hazards: a) North wall two (2) electrical outlets without cover plates; and b) South wall two (2) electrical outlets without cover plates.
