29 CFR 1910.146(e)(2): Before entry begins, the entry supervisor identified on the permit shall sign the entry permit to authorize entry. a. On or about May 25, 2018, an employee cleaning the inside of a semi-truck tanker was pronounced dead following a confined space rescue. The employee was found to have suffered chemical asphyxia by vitiated atmosphere, hydrogen sulfide and hydrogen cyanide gases. Where the hazardous atmosphere had not been eliminated prior to entry a permit-required confined space permit had not been signed by an entry supervisor.
