29 CFR 1910.212(a)(3)(ii): Point(s) of operation guards were not designed and constructed to prevent the operator from having any part of his/ her body in the danger zone during the operating cycle: On or about August 09, 2017, guarding to prevent employee's from having any part of the body in the danger zone during the operating cycle was not installed on the company's Cincinnati (90 ton, serial number 34583) press brake. The operator, helper(s) and others in the immediate work area are potentially exposed to pinch point hazards as the work stroke begins and the stock were to move up or down.
