29 CFR 1910.147(c)(7)(iii)(A): Retraining shall be provided for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures: a. The employer developed new procedures for lock-out/tag-out in 2015, and employees were not retrained on the new procedures, thereby exposing employees to caught in and electrical hazards. b. The employer made changes to the lock-out/tag-out program in 2014, 2015, and 2016 without retraining the employees that had been previously trained, thereby exposing employees to caught in and electrical hazards. c. The employer installed press lines 5007 and 5008 in 2014, without developing lock-out/tag-out procedures, thereby exposing employees to caught-in and electrical hazards. d. At the workplace, the employer did not develop, document or utilize procedures for the control of potentially hazardous energy when employees were clearing web jams and feeding paper webs into the N012 Harris Printing Press. e. At the workplace in the N012 area, the employer's lockout procedures for the N012 Enkel Splicer did not include a method to verify isolation and control of pneumatic energy of the N012 Enkel Splicer. The employer's procedures did not include instructions such as checking pressure gauges or bleeding of pneumatic lines. f. The employer does not perform periodic inspections of the energy control procedures on an annual basis, thereby exposing employees to a caught-in hazard. g. The employer failed to train new hire employees and temporary employees engaged in the press assistant II job to the affected level of lock out tag out, thereby exposing employees to a caught in hazard. h. The specific lockout steps were not completed for clearing jams from the folder, thereby exposing employees to a caught in hazard. i. On or about October 7, 2016, and times prior to, the specific lockout steps were not completed for employees who were hanging plates, thereby exposing employees to a caught in hazard. j. The specific lockout steps were not completed for cleaning blankets, thereby exposing employees to a caught in hazard. k. The specific lockout steps were not completed for changing belts on the folders, thereby exposing employees to a caught in hazard. l. The specific lockout steps were not completed for cleaning out the folder, thereby exposing employees to a caught in hazard. m. On or about November 15, 2016 and times prior to, the specific lockout steps were not completed for employees who bypassed the guard on the folders to adjust the guides while the fans were running, thereby exposing employees to a caught in hazard. n. On or about October 7, 2016, and times prior to, the specific lockout steps were not completed for employees who were clearing jams from the press lines, thereby exposing employees to a caught in hazard.
