29 CFR 1910.305(c)(4): Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface. A.) Presque Isle Wine Cellars, Inc., 9440 West Main Street, North East, PA 16428, chemical breakdown, on or about September 15, 2016 and times prior thereto: The cover plate for the snap switch box was broken. The damage consisted of a large hole on the left side of the box.
