29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. On or about June 21, 2016, in the Ammonia Refrigeration Process at American Cold Storage at Crossroads Distribution Center located in Tulsa, Texas, the employer did not ensure employees who are required to use tight-fitting facepiece respirators were fit tested at least annually.
