29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made: a) Fabrication Shop; On or about February 17, 2016 and at times prior thereto, an employee was exposed to an electric shock hazard in that the employee was using a De Walt brand portable band saw to cut square steel tubes that had the electrical cord outer insulation damaged.
