29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) BK Tires, Inc. at 3775 Puritan Way, Frederick, CO 80516: On or before July 16, 2015, the employer failed to provide information and training on the use of the Dowflake Xtra 83 - 87% Calcium Chloride Flakes, including protective measures and the personal protective equipment needed when handling this material, the hazards associated with inhalation of the dust, and exposure to the skin and eyes.
